1. ASSUMPTION OF RISK
I understand that Brazilian Jiu-Jitsu, Mixed Martial Arts, conditioning drills, and related activities at Land of Kingz, including training conducted in a private garage facility, involve inherent risks. These risks include, but are not limited to: sprains, strains, joint injury, head or neck injury, accidental collisions, serious injury, paralysis, or death. I voluntarily assume all risks associated with participation.
2. RELEASE OF LIABILITY
In consideration for participation, I hereby release and hold harmless Land of Kingz LLC, Professor Enrique Hernandez, coaches, assistants, volunteers, and property owners from any and all liability, claims, demands, or actions arising from participation, whether caused by negligence or otherwise. I understand that Land of Kingz is not responsible for medical expenses.
3. HEALTH & FITNESS DECLARATION
I certify that I (or my child) am physically able to participate and will not train while injured, ill, or contagious.
4. RULES & SAFETY AGREEMENT
I agree to follow all academy rules, including instructor direction, safe training practices, hygiene standards, and respectful conduct. Failure to comply may result in removal without refund.
5. MEDIA CONSENT (Check One)
